RN, LPN, and LVN Assessment/Intervention

At Shekina Glory Legacy Hospice and Palliative Services, we know that the initial patient assessment is crucial to providing effective care. That's why we have experienced RN, LPN, and LVN staff who will conduct a comprehensive initial assessment of the patient's condition. Our team will evaluate the patient's physical, emotional, and social needs and develop a care plan that meets their unique needs. After the initial assessment, our specialized staff will provide weekly follow-up visits to monitor the patient's progress and adjust the treatment plan as needed.
